If you are female:

- Read the information page of the request form.
- Complete the label on the information page with your name and date of birth and attach to the swab container.
- Tear off and keep the information page in a safe place. This has important information you will need to get your result.
- Fill in the request form. Make sure you give your name, date of birth and contact details, and at least the first part of your postcode otherwise your sample may not be tested. To protect your personal information, you may tear off the far right-hand section and destroy it.
- Take the test according to the instructions on the back of the information sheet.
- Put the sample into the clear plastic bag and seal.
- Return the sample and form to the testing site that gave you this kit, this must be done on the day you produce your sample.
